In addition to TELEHEALTH, the Transformation Counseling Office location remains OPEN and is focused on providing high quality services, while following all recommendations and mandates regarding COVID-19

To schedule an initial appointment at Transformation Counseling, with Joe, please call at 651-330-0144. During this call, we will go over a few details such as directions, insurance coverage, and goals in counseling.

For future appointments, please find a convenient time for you using the online scheduling tool provided, or you may schedule with Joe while in the office.

Intake forms are provided for you on the website to download, under “client forms.”  They are also provided in office (please allow 10-15 minutes at the initial visit in this case).
